Nominations for Parkinson's Award The central executive committee of World Parkinson's Education Program is pleased to honor outstanding individuals who are committed to improving the quality of life of patients with Parkinson's disease. Their dedication and hard work not only makes a difference in the quality of life of individuals with Parkinson's disease but also helps caregivers to better cope with stress and ease the burden of taking care of their loved ones. By recognizing those individuals who inspire others, World Parkinson's Education Program's dedication for community service continues through the work of others. The central executive committee of World Parkinson's Education Program requests subscribers of World Parkinson's Education Program and health care community workers to submit nominations for Dr. Rana Parkinson's Community Service Award. Organizations may nominate individuals who have consistently presented themselves as role models and leaders in Parkinson's community. 1. Nomination Recommendations Parkinson's community health volunteers Allied health care workers providing care to Parkinson's disease patients Physicians, Nurses, physiotherapists, other health care practitioners, organizers, committee and board members who play a pivotal role in this cause going above and beyond the call of duty. 2. The Nomination Process Nominations may be submitted by email at info@parkinsonseducation.org or completed nomination forms can be mailed at our postal address. The completed nomination forms must be received no later than March 31, 2012. Nomination Forms are available for download on our website www.parkinsonseducation.org. Nomination forms must be completely filled including the following information: Name, title, organization name (if applicable), address, telephone numbers and email address of both the nominator and nominee(s) Answers to all Award Criterion requirements Supplemental information that illustrates the nominees' contribution to the community. All nomination materials submitted will become the property of World Parkinson's Education Program and will not be returned. 3. Announcement of Recipient: Nominations will be reviewed by an Award Selection Committee, and the results will be announced on April 11, 2012, The World Parkinson's Day. The recipient will be informed via email provided. Award recipient will be required to submit a brief auto-biography and details of their work in the community. The Recipient's name will be announced in the electronic newsletter of World Parkinson's Education Program. The recipient's name will also be posted on the website of the program. The recipient has to agree with these conditions. The Award and Certificate will be mailed to the recipient.
