Research Participants Needed

I am a mother and a pediatric occupational therapist working in Minneapolis, MN. I am collecting data for a research project to help children with special needs better qualify for skilled therapy services and supportive services covered by their insurance companies.

Parents of typically developing children ages 2-18 years old are asked to take a one time online survey about their child's ability to complete self-cares and home and community skills. The link below will take you to the consent form, followed by a few demographic questions, and then to the survey. You may post this information on Facebook, blogs, and email it to family and friends. The questionnaire is multiple choice and does not ask any questions to identify you or your child to the data.

I am in especial need of data from parents of typical teens, 14-18 years old.
